One time use physical mailing labels of PT members.
Label Use Agreement:
The LIST RENTER acknowledges that the Minnesota Chapter American Physical Therapy Association (MN APTA) membership mailing lists and any portions thereof, are the exclusive property of the MN APTA, hereafter known as the LIST OWNER.
The LIST RENTER agrees and understands that all names and addresses furnished are provided for ONE-TIME USE only. The LIST RENTER guarantees the names and addresses shall not be copied, reused, sold, electronically reproduced, or used by any party except as specified in the written order.
On completion of each one-time mailing, the LIST RENTER shall immediately destroy all unused mailing labels, letters, envelopes, and other typed matter that contains names and addresses supplied by the LIST OWNER.
The LIST RENTER understands that MN APTA does not guarantee success of the LIST RENTER’S mailing, and cannot be held responsible for use of mailing list after the LIST RENTER receives said order.
If completed order is considered to be incorrect, the LIST RENTER must notify the LIST OWNER (MN APTA Office) within 30 days of receipt of order. After 30 days of receipt of order, the order is considered correct.
The LIST RENTER understands that there is a NO RETURN POLICY on label orders. If there are any doubts of how order will be perceived when processed, the LIST RENTER must contact the LIST OWNER (MN APTA Office) for clarification before placing the order. No refunds or credits will be made after an order has been processed.
The LIST RENTER agrees that the rental conditions herein will apply to any present and future rentals of the mailing list.
The LIST RENTER understands that there is a 7% margin of error per list for bad addresses for MN APTA Member addresses. No refunds or credits will be provided for returned labels below the 7% allowance.
Please Note: MN APTA’s Nonmember records are obtained in various ways, including purchasing other membership lists and nonmember attendance at conferences and CE courses. As MN APTA is not necessarily in regular contact with nonmembers, there is NO GUARANTY related to accuracy of nonmember records.
REFERRAL FOR PROFIT
APTA is opposed, as a matter of health care policy, to arrangements under which sources of referral (including physicians and chiropractors) stand to profit from referring patients for physical therapy. The policy, adopted by the APTA House of Delegates, states: “The American Physical Therapy Association opposes… participation in services that are in any way linked to the financial gain of the referral source.” (Financial Considerations in Practice HOD 06-99-13-17). Because of this policy, APTA and MN APTA does not accept mailing label requests from practices in which any referral source has a financial interest in the practice and refers patients to an employed physical therapist or to a physical therapist who supervises an employed physical therapist assistant.
